New Vendor / Contractor Requirements

Marsh Landing Management manages the association noted above, as directed by the Association's Board of Directors. Before the Association can contract with you or your firm, this form must be completed and returned along with the requested information and documents for our files.

Please forward the following identification and insurance information:

  1. W-9 - Required by IRS.
  2. Certificate of Liability and Property Damage Insurance and Certificate of Workers' Compensation Insurance (if work will be done on the association property}.
  3. The certificate holders name should be:

Invoices will be paid within 30 days after receipt. Each invoice should be submitted and contain the following information:

  1. An invoice number and complete description of work completed.
  2. If for a monthly service, the month for which the invoice is submitted.
  3. Location where work was done.
  4. Name of property manager authorizing work.

Acknowledgement

I, the undersigned, certify the above information is true and correct, and I understand and agree that
no payment for services or supplies will be made until all of the above requirements are met.
